First visit checklist

📄

Documents

  • Insurance card and photo ID
  • Referral or prior authorization (if required by your plan)
  • MRI, X-ray, or imaging results (digital copies are fine)
  • List of current medications
  • Completed intake forms (if sent ahead of time)
👟

Clothing and gear

  • Comfortable athletic clothing (shorts + T-shirt)
  • Supportive athletic shoes (no sandals or dress shoes)
  • Hair tie if your hair is long
  • Water bottle
  • Small towel (optional, for sweat)

Dress for the treatment area

What you wear matters because your therapist needs access to the area being treated. If your treatment is for a shoulder injury, a tank top or sleeveless shirt lets them work without constantly adjusting fabric. Knee or hip treatment? Shorts that sit above the knee.

Avoid jeans, belts, heavy layers, or anything that restricts movement. You'll be bending, stretching, and moving through your full range of motion. Dress like you're going to the gym, not a doctor's office.

What to bring to follow-up sessions

Follow-ups are lighter. You've already done the paperwork. Just bring:

Tracking your home exercises and pain levels between sessions gives your therapist real data to adjust the plan. A simple notes app or paper log works.

A reminder gives you time to prepare

The difference between showing up ready and showing up scrambling is usually a day of notice. A reminder 1 to 3 days before your PT session gives you time to wash your gym clothes, gather any notes, and confirm with the clinic.

Questions about preparing for physical therapy

What should I wear to physical therapy?

Wear comfortable, loose-fitting athletic clothing that allows full range of motion. Shorts and a T-shirt work for most sessions. If your treatment area is your shoulder, wear a tank top or sleeveless shirt. Avoid jeans, belts, and restrictive clothing.

What shoes should I wear to physical therapy?

Wear supportive athletic shoes with good grip. Running shoes or cross-trainers are ideal. Avoid sandals, flip-flops, or dress shoes. If your treatment involves gait training or lower body work, proper footwear is especially important.

Do I need to bring my insurance card to physical therapy?

Yes, especially for your first visit. Bring your insurance card, a photo ID, and any referral or prior authorization paperwork from your doctor. The front desk verifies coverage before your first session.

Should I bring my MRI or X-ray results to physical therapy?

If you have imaging results, bring them. Physical therapists can treat without imaging, but having your MRI, X-ray, or CT scan results helps them understand the full picture and tailor the treatment plan. Digital copies on your phone work fine.

What should I avoid doing before physical therapy?

Avoid heavy meals right before your session. Light eating is fine. Stay hydrated. Skip intense exercise the morning of your appointment if you are in early-stage rehab. Do any prescribed home exercises the day before, not right before your session.

Do I need to bring anything to follow-up PT appointments?

For follow-ups, just bring yourself, your shoes, and a water bottle. If your therapist asked you to track home exercises or pain levels, bring those notes. You do not need to repeat the paperwork process after the first visit.
